Invitations and sign-in

Use the latest invitation for your intended team on a device with the correct FormAthX build. Invitation and recovery links expire and are single-use. Do not forward them. If a link fails, return to sign-in and request a fresh link through the authorized administrator or password recovery flow.

Forgot your password?

Choose Forgot password, request a new email and open only the newest recovery link. Check spam and expand collapsed email-thread content. Recovery changes your password; it does not grant a team membership or a new role.

A change is not visible

Check the account, active team, app version and environment. Successful writes trigger refresh; screens also refetch on focus and foreground, with scoped live updates on selected operational screens. Offline information may be stale. A timeout is not proof that a save failed: use the built-in retry rather than duplicating a record.

Who can see which information?

Active same-team Sport Therapists have authorized clinical access. Players receive their permitted self-service records and limited injury summaries, not clinical document attachments. Coaches receive permitted training, review and participation information, not unrestricted clinical details. Club/team administration or Platform Owner authority alone does not grant clinical access.

Test-result documents

The locally tested candidate accepts PNG/JPEG scans for an authorized same-team Sport Therapist, with a 2 MiB and 4-million-pixel limit. The server stores a metadata-stripped PNG copy. PDF and office files are not supported. Production deployment and signed-device testing remain pending; do not assume this candidate feature is available on the live backend.

Phone notifications

Production notification delivery is not enabled or physically verified. The candidate supports an in-app inbox and optional phone permission. Declining phone notifications must leave ordinary app access available. Push text must not contain diagnoses, pain scores or treatment notes. Provider acceptance is not proof of delivery to a phone.

Medical and emergency boundaries

FormAthX records athlete reports and human staff decisions. It does not diagnose, provide emergency monitoring, automatically clear an athlete or decide return to sport. Contact an appropriate professional or local emergency service directly for urgent needs.

Requesting account deletion

The candidate app includes Profile → Delete account to initiate a request and follow its status. Signing out or archiving is not account deletion. A request or receipt is not proof that deletion is complete. The compatible Production server, retention decisions and operational completion process are still pending; this is not a claim that live deletion or export has been verified. Contact support if you need help; do not email a password, sign-in link or health document.

What to include in a support request

Email support@formathx.com with the device, OS, app version/build, approximate time, screen and error text. Never send a password, verification code, sign-in link, push token or health information. Email receipt does not verify manual reply-as-domain configuration, account export, completed deletion or delivery of an automated completion notice.
